Typesetting Questionnaire

Authors usually have a consistent style applied to all their books.
A series also usually have a consistent style.
A common trim size is 6 x 9 inches (152.4 x 228.6 mm)
Paperbacks and hardcovers = 300dpi. Ebooks = 96 to 150dpi.
An index is different from a table of contents. The index is at the back of the book and it lists the page numbers of important topics.
Folios = page numbers. Running headers = small ALL CAPS book titles on the top of the left page, and small ALL CAPS chapter headings on top the right page.
K = digital or litho printing. RGB = digital printing. CMYK = litho printing.
